Community Involvement Programs (CIP) recognizes the type of care and support individuals want varies. Through our Community Services Team we strive to provide a person-centered approach to services in the community or within the individuals home. We offer a variety of programs, including those for individuals with intellectual disabilities, traumatic brain injury or severe and persistent mental illness. Our goal is to empower participants, and their family members and supporters to use their resources to make informed decisions.

Explore Our Community Service Options

We currently provide the following as a part of Community Services:

Independent Living Skills (ILS): The mission of ILS is to empower participants to take on more responsibility for day-to-day living. We provide support, training, assistance and even supervision to help participants achieve their goals of self-reliance.

Personal Support Services: A waiver funded program, created to increase independence and inclusion in the community, PSS is a service that allows individuals to choose the staff that support them through a variety of services.

Pine County Community Supports: Staff provide supports in the homes of participants to assist with day-to-day living, within Pine County.

Supportive Housing: These affordable housing opportunities provide individuals with mental illness a place to live. Support Services provided by a mental health worker are provided in Shared Housing. Other Services may be an option for people in Individualized Housing.

Support Planning: Our staff use their knowledge with resources, advocacy, and person-centered practices to offer individuals support with the development of their CDCS plans.

Supported Living Partnerships (SLP): Individuals are supported by staff who work in the homes and community with the participant to support day-to-day living, within Hennepin County.
