replace_homehealthDetermining the medication which will help stabilize a person’s mental health symptoms and cause the least amount of side effects is often a big challenge for people living with a mental health diagnosis. CIP’s experienced psychiatric nurses will work with participants and their psychiatrists to evaluate which medications are most effective for them.

What do visiting nurses do?

We will also develop ways to increase participant’s compliance in taking their medications and improve their participation with their psychiatric and medical providers. The registered nurses provide individual support to people with a mental health diagnosis in the community, some residing in their own homes and also in CIP’s Supportive Housing and Adult Foster Care Homes. Our skilled nursing services are offered through our Medicare certified Home Health Agency.

In addition to assisting people with medication management, nurses complete a mental health assessment at each visit, to evaluate people’s stability. They will develop individual crisis plans to utilize should the person experience a period of instability. Nurses will monitor and assist participants with chronic medical conditions and will communicate and advocate people’s needs with their providers.

CIP contracts with a local pharmacy and has a full time pharmacy technician on site. Nurses will order, obtain and deliver medications with their visits. When appropriate nurses may also set up a one week supply of medications and keep the remainder of the medication supply. In some situations, the CIP Home Health nurse may complete skilled visits for the purpose of administering psychotropic injections.

Where are services offered and how are they paid for?

CIP’s Home Health Agency services are currently available in Hennepin, Ramsey, Anoka, Washington, and Scott counties. Services are provided through Medicaid (fee for service)- Medica- U-Care and MHP. Participants may also self- pay.
